    Yes! Your treatment choices are up to you. You can receive palliative care at the same time as treatment meant to cure you or simply maintain an optimum level of health.

    Unlike hospice care, which is limited to the last six months of life, palliative care is appropriate at any age and at any stage of a serious illness. It can be provided along with treatment intended to cure your illness.

Our team accomplishes these goals through expert symptom management, education of the disease, connection with community resources (based on psychosocial needs), and assessment of goals to guide your plan of care. It can be provided along with current treatments designed to cure and/or support your illness.

    Our patients will continue to receive the following:

      • In-Home and/or virtual Nurse Practitioner and Physician visits
      • In-Home and/or virtual Palliative Care Social Work visits
      • Virtual/Telephone Nurse care coordination and medication management
      • 24/7 telephone access to providers for palliative care needs
      • Medication management for symptom medications
      • Assistance with advance care planning and advance directives
