    Yes! Your treatment choices are up to you. You can receive palliative care at the same time as treatment meant to cure you or simply maintain an optimum level of health.

    • Does insurance cover palliative services?

    Palliative care services are covered by most insurance plans. Financial assistance is also available for those unable to pay. Please contact us to discuss your needs and let us help you determine coverage eligibility and fees.

    • Is palliative care the same as hospice care?

    Unlike hospice care, which is limited to the last six months of life, palliative care is appropriate at any age and at any stage of a serious illness. It can be provided along with treatment intended to cure your illness.

  • Palliative care is patient-centered medical care for anyone facing a serious illness. The goal is to improve your quality of life by focusing on relief from symptoms and the stress of the illness. Our team accomplishes these goals through expert symptom management, education of the disease, connection with community resources (based on psychosocial needs), and assessment of goals to guide your plan of care. It can be provided along with current treatments designed to cure and/or support your illness.

    Our patients will continue to receive the following:

      • In-Home and/or virtual Nurse Practitioner and Physician visits
      • In-Home and/or virtual Palliative Care Social Work visits
      • Virtual/Telephone Nurse care coordination and medication management
      • 24/7 telephone access to providers for palliative care needs
      • Medication management for symptom medications
      • Assistance with advance care planning and advance directives

    • Does not qualify for hospice care
    • Suffers from pain or other symptoms due to ANY serious illness that is not well-controlled

    • Needs help understanding the health situation and coordinating care
