Pallitus Health Partners is a part of the Hosparus Health family but separates our palliative care services from hospice services. We offer an extra layer of support for anyone facing serious illness, providing expert care that is designed to help you make the most of each day.

Our support extends to the patient’s family, caregivers and other loved ones. Pallitus Health Partners is committed to helping people in our community live better, longer. We truly believe that the earlier you call, the more we can help.

Who Our Care is Centered On

We serve patients with serious illnesses such as cancer, heart failure, COPD, etc. whose disease has significantly impacted your daily living and functional abilities, and puts you at risk for increased ER visits and hospitalization.

We enhance the quality of life for patients and their families by managing symptoms, educating you about your disease and its trajectory, connecting you with the appropriate community resources, and assessing your goals to guide your plan of care. Pallitus Health team members provide care wherever you call home and work in coordination with your other doctors to help you navigate all the challenges of serious illness — physical, medical, social and emotional.

Pallitus Health Partners cares for patients of all ages and backgrounds – regardless of race, religion, ethnicity, gender identity, sexual orientation, age, handicap or ability to pay. Care is delivered through a team-oriented approach and is tailored to each patient’s needs and wishes.

Interpretation and Translation Services

Pallitus Health Partners provides interpreters to communicate with patients who have significant hearing loss or who cannot speak, read, write or understand the English language at a level that permits them to interact effectively with our staff members. Pallitus Health Partners also will provide all vital documents in the requested primary language. These services are provided free of charge.
