We offer comprehensive palliative care across several lines of services, including specialized care for chronic symptoms and medication management, disease education, and care coordination. Teams of highly trained caregivers bring medical expertise and support so that you can enjoy the highest quality of life during any stage of serious illness.

What to expect in palliative care.

Palliative care is appropriate at any age and at any stage of a serious illness such as cancer, heart failure, COPD, dementia, Parkinson’s disease, etc. It can be provided along with current treatments designed to cure and/or support your illness. All services are offered wherever you call home and for as long as you need us.

 

Pain and Symptom Management

A primary goal of our care is to ensure your comfort and to assist with symptom management. During each visit we will check the your pain level and adjust accordingly. You can expect your care team to make recommendations to address your symptoms and decrease distress. You will also receive expert guidance from our palliative care team members on improving symptoms and impacting your quality of life.

Medication Management

Treating complex illnesses requires complex medication management. With the guidance of your existing doctors and specialists, we ensure that you are getting the right doses of medication and that your symptoms are managed effectively.

 

Social Work Visits

In addition to visits with a nurse practitioner and physician, you may also receive in-home and/or virtual visits with a social worker that may connect you with community resources (based on psychosocial needs). Our social workers are also equipped to provide advance care planning.

24/7 Phone Support

We provide around-the-clock phone support for everyone. This service is offered at no added cost.

Nurse Practitioner and Physician visits

We have one of the largest teams in the country of doctors and nurse practitioners who are board-certified in hospice and palliative medicine. Our team will work in coordination with your existing healthcare providers to ensure your care aligns with your treatment goals. Our NP and Physicians will also help provide a better understanding of your disease and prognosis. These visits are available in-home and/or virtual.

Advance Care Planning

Pallitus Health encourages you to make future healthcare decisions known to family, friends, and healthcare providers. We can provide assistance with planning for your future healthcare needs, including completion of advance directives
